背景技術(shù):
口腔頜面部惡性腫瘤是常見(jiàn)高發(fā)的頭頸部惡性腫瘤,容易發(fā)生頸部淋巴結(jié)轉(zhuǎn)移,預(yù)后較差。研究表明,這種腫瘤的發(fā)生發(fā)展是多基因參與、多步驟、多階段的復(fù)雜過(guò)程,其中抑癌基因的失活與癌基因的激活發(fā)揮了關(guān)鍵的作用。
長(zhǎng)鏈非編碼RNAs(long non-coding RNAs,lncRNAs)是一類(lèi)長(zhǎng)度超過(guò)200nt、缺少特異完整的開(kāi)放閱讀框、無(wú)或很少有蛋白編碼功能的RNAs分子。最近的研究表明,lncRNAs通過(guò)在表觀遺傳、轉(zhuǎn)錄和轉(zhuǎn)錄后水平廣泛調(diào)節(jié)基因的表達(dá),它們參與了生物體生長(zhǎng)、發(fā)育、衰老及死亡等重要生命活動(dòng)的調(diào)控。越來(lái)越多的研究表明lncRNAs的異常表達(dá)或功能缺失與腫瘤的發(fā)生發(fā)展密切相關(guān)。一些lncRNAs分子在腫瘤的診斷和治療方面具有重要的意義,且可以作為腫瘤預(yù)后判斷新的分子標(biāo)記。目前,已經(jīng)克隆的人類(lèi)lncRNAs基因已超過(guò)4萬(wàn)多個(gè),但絕大部分lncRNA的功能未知。
我們檢測(cè)了LINC00472在臨床離體腫瘤樣本中的表達(dá)水平,并分析了LINC00472表達(dá)水平與臨床病例資料的相關(guān)性。我們從口腔腫瘤及相對(duì)應(yīng)的正常對(duì)照組織中抽提RNA后通過(guò)逆轉(zhuǎn)錄,實(shí)時(shí)定量PCR,檢測(cè)了LINC00472的表達(dá)情況,結(jié)果顯示LINC00472在口腔腫瘤組織中表達(dá)下調(diào)。我們隨后又利用原位雜交的方法,進(jìn)一步在存檔石蠟樣本驗(yàn)證了LINC00472在口腔腫瘤中表達(dá)顯著下調(diào),因此針對(duì)LINC00472的檢測(cè)制劑可用于口腔腫瘤的輔助診斷,結(jié)合臨床回訪資料進(jìn)行生存曲線分析發(fā)現(xiàn)LINC00472表達(dá)低的患者其生存時(shí)間短于該lncRNA表達(dá)高的患者,因此針對(duì)該lncRNA的檢測(cè)制劑也可以用于口腔腫瘤的預(yù)后判斷。
技術(shù)實(shí)現(xiàn)要素:
本發(fā)明的目的是提供檢測(cè)口腔腫瘤組織中長(zhǎng)鏈非編碼RNA LINC00472表達(dá)量的試劑的應(yīng)用,所述的試劑用于制備口腔腫瘤輔助診斷或者療效預(yù)測(cè)制劑,該長(zhǎng)鏈非編碼RNA LINC00472的序列見(jiàn)SEQ NO:1。
所述的口腔腫瘤輔助診斷或者療效預(yù)測(cè)制劑是根據(jù)長(zhǎng)鏈非編碼RNA LINC00472在口腔腫瘤中的表達(dá)情況來(lái)進(jìn)行輔助診斷和療效預(yù)測(cè),口腔腫瘤組織中LINC00472表達(dá)顯著下調(diào)。
所述的口腔腫瘤輔助診斷或者療效預(yù)測(cè)制劑為實(shí)時(shí)熒光定量檢測(cè)試劑。
所述的實(shí)時(shí)熒光定量檢測(cè)試劑中包含
用于實(shí)時(shí)熒光定量檢測(cè)LINC00472表達(dá)的引物序列:
正向引物:5’-GCATCTGTCAACGCTCCTCT-3’,
反向引物:5’-CCGCGTTCTTTGTGTGTCTC-3’。
所述的實(shí)時(shí)熒光定量檢測(cè)制劑包含:
內(nèi)參基因GAPDH特異性PCR引物:

試劑中還含有:
(1)從口腔腫瘤組織中抽提總RNA所用試劑,包括RNA穩(wěn)定溶液、Trizol試劑、三氯甲烷、異丙醇、無(wú)酶水;
(2)以總RNA為模板將LncRNA LINC00472逆轉(zhuǎn)錄為cDNA所用試劑,包括逆轉(zhuǎn)錄緩沖液、三磷酸堿基脫氧核苷酸、RNA酶抑制劑、MMLV逆轉(zhuǎn)錄酶以及隨機(jī)引物;
(3)將cDNA實(shí)時(shí)定量PCR所用試劑,包括實(shí)時(shí)熒光定量SYBR染料、無(wú)酶水。
我們從口腔腫瘤及正??谇簧掀颖局谐樘酭NA后逆轉(zhuǎn)錄,實(shí)時(shí)熒光定量法檢測(cè)了LINC00472的表達(dá),結(jié)果顯示LINC00472在口腔腫瘤組織中表達(dá)下調(diào)。提示LINC00472可作為口腔腫瘤預(yù)后預(yù)測(cè)的分子標(biāo)志。本發(fā)明為口腔腫瘤的輔助診斷和預(yù)后預(yù)測(cè)提供了強(qiáng)有力的分子生物學(xué)工具,具有深遠(yuǎn)的臨床意義和重要的推廣應(yīng)用前景。

實(shí)施例1,實(shí)時(shí)熒光定量法檢測(cè)證實(shí)LINC00472在口腔腫瘤中表達(dá)下調(diào)
1.材料與方法:
9例正??谇徽衬ず?例口腔腫瘤組織抽提總RNA,2μg RNA經(jīng)逆轉(zhuǎn)錄成cDNA后,進(jìn)行實(shí)時(shí)熒光定量PCR。
正向引物:5’-GCATCTGTCAACGCTCCTCT-3’,
反向引物:5’-CCGCGTTCTTTGTGTGTCTC-3’。
用于對(duì)照的內(nèi)參看家基因GAPDH特異性PCR引物:
正向引物5’-ACCACAGTCCATGCCATCAC-3’

實(shí)時(shí)熒光定量PCR反應(yīng)步驟
反應(yīng)結(jié)束后確認(rèn)實(shí)時(shí)熒光定量PCR的擴(kuò)增曲線和熔解曲線,各基因的表達(dá)強(qiáng)度根據(jù)CT值(threshold cycle values)、內(nèi)參基因(GAPDH)標(biāo)化后,采用group t-test檢驗(yàn)計(jì)算P值。
2.結(jié)果
LncRNA LINC00472在正常對(duì)照組織中表達(dá)較低,而在口腔腫瘤組織中低表達(dá)或者不表達(dá)P<0.001(圖1)
實(shí)施例2,原位雜交檢測(cè)發(fā)現(xiàn)LINC00472在口腔腫瘤中的表達(dá)與患者預(yù)后相關(guān)
1.材料方法
1.1用于口腔腫瘤和正常對(duì)照組織原位雜交的寡核苷酸探針
根椐LINC00472基因序列分析設(shè)計(jì)出雜交參數(shù)最佳特異性最好的3條寡核苷酸探針,另外,以看家基因GAPDH為陽(yáng)性對(duì)照。
用于原位雜交檢測(cè)LINC00472表達(dá)的寡核苷酸探針:
LINC00472探針1:5’-CAAAACTAGGATTCTGTCTTTGAGGCAGAC-3’
LINC00472探針2:5’-AAGAATGGAAGTGGGAAGAGAGAAAGACTT-3’;

陽(yáng)性對(duì)照探針(檢測(cè)看家基因GAPDH):

寡核苷酸探針采用化學(xué)合成方法合成
1.2寡核苷酸探針標(biāo)記試劑盒和原位雜交檢測(cè)試劑
Dig Oligonucleitide Tailing Kit(2nd Generation)試劑盒(Roche公司),抗地高辛-辣根過(guò)氧化物酶復(fù)合物檢測(cè)試劑盒(Anti-Digoxigenin-POD,F(xiàn)ab fragments,Roche公司)。增強(qiáng)原位表達(dá)檢測(cè)信號(hào)的TSA信號(hào)放大系統(tǒng)(TSATM Biotin System,NEL700試劑盒,PerkinElmer公司)。DAB染色試劑盒(北京中山公司)。20×SSC,硫酸葡聚糖(Dextran sulphate),去離子甲酰胺(Deionized Formamide),多聚腺苷酸(polyadenylic acid,Poly A),多聚脫氧腺苷酸(polydeoxyadenylic acid,Poly dA),變性剪切的蛙精DNA(denatured and sheared salmon spermDNA,ssDNA),酵母轉(zhuǎn)運(yùn)RNA(yeast t-RNA,tRNA),DTT,50×Denhardts’s solution,PBSbuffer,胃蛋白酶K,BSA(牛血清清蛋白),三乙醇胺(TEA),TNB Buffer(0.1M Tris-HCl,pH7.5,0.15M NaCL,0.5%Blocking Reagent),TNT Buffer(0.1M Tris-HCl,pH7.5,0.15M NaCL,0.05%Tween 20)醋酸酐,阻斷試劑(Blocking reagent agent,Roche公司)。
1.3其他主要試劑和材料
無(wú)水乙醇、90%酒精、70%酒精、50%酒精、松節(jié)油、雙蒸水、PBS緩沖液(pH7.2~7.4,NaCl 137mmol/L,KCl 2.7mmol/L,Na2HPO4 4.3mmol/L,KH2PO4 1.4mmol/L);3%甲醇-雙氧水溶液(80%甲醇和30%雙氧水配置);0.01mol/L檸檬酸鹽緩沖液(citrate buffer,CB,pH6.0±0.1,9ml 0.1M檸檬酸溶液和41ml 0.1M檸檬酸鈉溶液加入450ml蒸餾水中臨時(shí)配置后再校正工作液pH值);0.1%胰蛋白酶;蘇木素;1%鹽酸酒精(1ml濃鹽酸+99ml 70%酒精配置);組織微陣列專用封片膠(PTS Cure MountⅡ);專用蓋玻片(480×240mm2)定制于鄭州玻璃儀器廠。Leica低熔點(diǎn)(58℃)石蠟,國(guó)產(chǎn)蜂蠟,無(wú)水酒精,二甲苯,10%中性多聚甲醛(0.01mol/L,pH7.4,DEPC雙蒸水和PBS緩沖液配制),蘇木素,伊紅,中性封片樹(shù)膠,蓋玻片,載玻片。
1.4寡核苷酸探針的標(biāo)記
利用3-tailing DIG Olignucleutide Kit進(jìn)行寡核苷酸探針標(biāo)記,反應(yīng)體系如下。
100pmol oligonucleotide+ddH2O=9μl(control:control oligonucleutide 5μl+ddH2O 4μl)
混勻,稍離心。37℃水浴反應(yīng)30min,加2ul EDTA(0.2M,pH 8.0)中止反應(yīng)。
1.5寡核苷酸探針標(biāo)記后純化
為了增加標(biāo)記探針的純度,需對(duì)已標(biāo)記的探針進(jìn)行純化,具體操作如下:
a)探針?lè)磻?yīng)混合物(22μl)+2.5μl 4M LiCL+75μl 100%冷乙醇(-20℃).
b)-70℃沉淀60min,or-20℃2h。
c)13.000×g 4℃離心15min。
d)棄上清,用50μl冰冷的70%(V/V)乙醇洗滌。
e)13.000xg 4℃,離心5min。
f)棄上清,真空4℃干燥。
g)用無(wú)菌雙蒸水重溶探針。
1.6組織切片雜交前處理
a)4℃保存的組織切片置于58℃烤片30min,熔化表面石蠟。
b)二甲苯依次脫蠟3×5min。
c)梯級(jí)酒精洗滌,100%酒精2×2min→95%酒精1×5min→70%酒精1×5min→50%酒精1×5min→DEPC水洗滌2×3min→DEPC-PBS洗滌2×5min。
d)滴加300μl胃蛋白酶K(10μg/ml)于切片上,37℃消化20min。
e)切片入PBS(0.1M PBS+2mg/ml谷氨酸)洗滌1min,中止反應(yīng)。
f)切片入0.2N HCL,于37℃反應(yīng)20-30min,增加組織的通透性。
g)切片用4%多聚甲醛(0.1M PBS溶解)后固定10min,室溫。
h)為了增加組織陽(yáng)性雜交強(qiáng)度,對(duì)切片進(jìn)行乙酰化處理。切片入0.25%乙酸酐BufferⅠ(0.1M三乙醇胺),室溫10min。
i)1M PBS洗滌2×5min。
1.7組織預(yù)雜交和雜交
a)預(yù)雜交:-20℃保存的預(yù)雜交液,先置于37℃孵育60min,預(yù)雜交液的用量為每平方厘米切片面積12.5μl,相應(yīng)大小的石蠟?zāi)じ采w切片,37℃濕盒中預(yù)雜交2小時(shí)。(預(yù)雜交液成份包括:2XSSC,10%Dextran sulphate,1X Denhardt’s solution,50mM Phosphate Buffer(PH 7.0),50mM DTT,250μl,100μg/ml poly A,5μg/ml poly dA,250μg/ml yeast t-RNA,500μg/ml ssDNA,47%Deionized formamide)。
b)移去石蠟?zāi)?,甩掉預(yù)雜交液,切片置于2×SSC中5min。
c)雜交反應(yīng):37℃雜交過(guò)夜(18-20h)。每一切片加入250μl雜交液并用石蠟?zāi)じ采w。預(yù)雜交液中加入相應(yīng)的探針就成為雜交液。雜交液在預(yù)雜交時(shí)配制,放置37℃孵育,使探針充分溶解于雜交液中,本實(shí)驗(yàn)用多條寡核苷酸探針混合,按每一探針500ng/ml濃度配制成探針雜交液。地高辛加尾標(biāo)記試劑盒標(biāo)記探針濃度計(jì)算依據(jù):每一探針的濃度按其與陽(yáng)性定量探針時(shí)檢測(cè)反應(yīng)時(shí)顯色進(jìn)行比對(duì)和100pmol 30個(gè)堿基的裸探針標(biāo)記反應(yīng)理論探針產(chǎn)量為900ng兩種標(biāo)準(zhǔn)進(jìn)行綜合計(jì)算出標(biāo)記探針的濃度。
d)雜交后洗滌,切片浸入2×SSC,10min,揭去石蠟?zāi)?。依次于搖床上搖動(dòng)洗滌,2×SSC(0.5%SDS),2×15min→0.25×SSC(0.5%SDS),2×15min。
1.8雜交后顯色檢測(cè)反應(yīng)
a)采用Anti-Digoxigenin-POD檢測(cè)地高辛探針與目的RNA結(jié)合復(fù)合物;TSA放大系統(tǒng)增強(qiáng)原位雜交反應(yīng)顯色反應(yīng)的陽(yáng)性信號(hào),DAB顯色。
b)切片轉(zhuǎn)至TNT緩沖液中,3×5min。
c)滴加TNB阻斷緩沖液,300μl/TMAs,室溫,30min。
d)不洗,吸去多余阻斷劑,1:100稀釋的Anti-Digoxigenin-POD(TBS+0.1%TritonX-100+1%阻斷劑),室溫4小時(shí)。
e)TNT Buffer(0.1M Tris-HCl,PH7.5,0.15M NaCL,0.05%Tween 20)洗3×5min。
f)切片上滴加信號(hào)放大試劑Biotinyl Tyamid,300μl/TMAs,(Biotinyl Tyramid貯存液:Biotinyl Tyramid溶解于0.2ml DMSO,Biotinyl Tyramid工作液:1×稀釋液,1:50稀釋Biotinyl Tyramid貯存液),室溫10分鐘。
g)TNT洗,3×5min。
h)切片滴加SA-HRP(鏈霉卵白素-辣根過(guò)氧化物酶),300μl/TMAs,室溫30min。
i)TNT洗,3×5min。
j)蒸溜水洗滌,1×1min。
k)DAB顯色,顯微鏡下控制顯色反應(yīng)。
l)蘇木素復(fù)染,
m)酒精梯級(jí)脫水,切片干燥。
n)滴加組織切片專用封片膠,相應(yīng)規(guī)格的蓋玻片蓋片,切片膠帶轉(zhuǎn)移系統(tǒng)配備的紫外燈下交聯(lián)切片1min。
1.9結(jié)果判斷及標(biāo)準(zhǔn)
應(yīng)用光學(xué)顯微鏡分別在低倍和高倍鏡下進(jìn)行觀察,首先觀察目標(biāo)RNA的陽(yáng)性表達(dá)信號(hào)在觀察目標(biāo)細(xì)胞內(nèi)的定位:位于細(xì)胞核、細(xì)胞漿或細(xì)胞膜。
再分別以該檢測(cè)RNA表達(dá)部位陽(yáng)性信號(hào)的強(qiáng)度和陽(yáng)性表達(dá)的細(xì)胞數(shù)兩種標(biāo)準(zhǔn)進(jìn)行綜合評(píng)分,判斷標(biāo)準(zhǔn)為:(1)依據(jù)陽(yáng)性細(xì)胞染色強(qiáng)度判斷:a.細(xì)胞無(wú)染色,記0分;b.細(xì)胞染成淺棕色為弱陽(yáng)性,記1分;c.細(xì)胞染成棕色且無(wú)背景著色,或細(xì)胞染成深棕色并有淺棕色背景為中等陽(yáng)性,記2分;d.細(xì)胞染成深棕色且無(wú)背景著色為強(qiáng)陽(yáng)性,記3分。(2)依據(jù)陽(yáng)性細(xì)胞表達(dá)數(shù)計(jì)分:a.無(wú)陽(yáng)性細(xì)胞表達(dá),記0分;b.陽(yáng)性表達(dá)細(xì)胞數(shù)≤25%,記1分;c.25%<陽(yáng)性細(xì)胞數(shù)<50%,記2分;d.陽(yáng)性表達(dá)細(xì)胞數(shù)≥50%,記3分。
為了盡量降低評(píng)分結(jié)果的主觀因素,由兩位病理學(xué)專家分別按上述標(biāo)準(zhǔn)之一各自進(jìn)行判斷和評(píng)分,再將兩者評(píng)分相乘,結(jié)果為:①0分者最終計(jì)為0分,認(rèn)為陰性表達(dá);②1分和2分者最終計(jì)為1分,認(rèn)為弱陽(yáng)性表達(dá);③3分和4分者最終計(jì)為2分,認(rèn)為中等陽(yáng)性表達(dá);④6分到9分者最終計(jì)為3分,認(rèn)為強(qiáng)陽(yáng)性表達(dá)。
1.10分析和統(tǒng)計(jì)軟件
應(yīng)用SPSS13.0統(tǒng)計(jì)軟件對(duì)實(shí)驗(yàn)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析,兩兩比較用χ2test或Fisher exact test,相關(guān)性分析采用Spearmen correlation方法;P<0.05即差異有統(tǒng)計(jì)學(xué)意義。生存曲線分析采用Kaplan-Meier method及l(fā)og-rank test;多變量分析采用Cox’s proportional hazards model;P<0.05即差異有統(tǒng)計(jì)學(xué)意義。
2結(jié)果
1)lncRNA LINC00472在口腔腫瘤及正常對(duì)照組織中的表達(dá)
LINC00472在正常對(duì)照組織中均有表達(dá)(圖2左),而在大部分口腔腫瘤組織中表達(dá)較低或不表達(dá)(圖2右),兩者之間具有明顯的統(tǒng)計(jì)學(xué)差異(P<0.05)。
2)Kaplan-Meier生存分析
我們對(duì)所有69例口腔腫瘤患者進(jìn)行了電話隨訪,詳細(xì)詢問(wèn)了他們的首發(fā)時(shí)間、治療情況、有無(wú)復(fù)發(fā)、有無(wú)再患其他疾病、復(fù)發(fā)及死亡時(shí)間等,并登記了生存時(shí)間和狀態(tài),并對(duì)口腔腫瘤組織中LINC00472的表達(dá)與病人的生存時(shí)間和狀態(tài)進(jìn)行的生存分析,發(fā)現(xiàn)LINC00472高表達(dá)的患者生存時(shí)間較LINC00472表達(dá)低或者不表達(dá)的患者要長(zhǎng)(圖3)。說(shuō)明LINC00472是一個(gè)與口腔腫瘤預(yù)后相關(guān)的分子標(biāo)記,該lncRNA表達(dá)低,病人預(yù)后差。
